Long Term Care Resource Guide

" What happens after Medicare
pays the first 20 days in a
Nursing Home Rehabilitation Center? "

Your Options Are:

A.  NURSING HOME

Your next 80 days Medicare pays 80% of which the residents portion to pay averages $105 per day if no supplemental insurance exists and if the resident meets Medicare criteria.  After the total of 100 days, either the resident pays privately at an average of $330 per day or is paid by Medicaid (only if the meet the Medicaid criteria).  (You should speak to a nursing home admissions coordinator for further information).  To qualify for Medicaid, the resident needs to have their total assets under $3500 for a 36 month look back period.  (Differs for married couples).

B.  HOME CARE

This is typically a costly option if not on Medicaid and is approximately $207 for a 24 hour period for those paying privately.  The costs of the person's home and living expenses still remain and there may not be as much personal contact as in a congregate setting.

CLICENSED ASSISTED LIVING  (ADULT CARE HOMES)

These homes are fully regulated by the NYS Department of Health.  Each facility is unique to the type of care and services provided.  There are facilities who focus on residents with a mental health history and other facilities which care for elderly residents who need more attention, minimal assistance and/or no assistance.  Medicaid does not apply in this level of care at this point but is anticipated.  Currently government assistance is through SSI at $1007 per month.  The resident's family can supplement the SSI amount to meet the facilities rate.   Similar requirements apply to qualify for SSI as with Medicaid with the residents total assets would need to be under $2000.  HOWEVER, this funding is immediately available upon admission to the facility with no look back period.  Smaller and older Assisted Living or Adult Home rates typically range from SSI to $2800 per month.  Newer and larger licensed facilities may range from $1500 - $4500 per month.

BIRCHWOOD ASSISTED LIVING
is a mid sized licensed adult care facility that can care for greater health needs if required.  Rates range from $1800 - $3800 per month.  A resident with SSI would be accepted with an additional third party payment.  There is a physical rehabilitation center on-site that Medicare B will pay for ongoing therapy needs and also may be paid through private insurance.  Ideal for rehabilitation after the 100 days.

D.  UNLICENSED ASSISTED LIVING

These facilities are NOT under NYS Department of Health inspection authority and the care is subcontracted through a separate licensed home care agency.  Typically these facilities give less attention to care and more on hospitality services.  Care services typically are a-la-cart and when offered the full services that a licensed assisted living give, their rates range on average from $4000 - $7000 per month for the basic room and care services.
